The Comprehensive Care Team (CCT)The Comprehensive Care Team is a person-centered approach which assures the mind, body and spirit of each resident is considered in the course of their daily care. This team is comprised of specially trained personnel who collaboratively structure a resident's personal care and psychosocial needs in a manner that provides consistency and structure. This team is comprised of a Residence Manager who will work hands on with residents to determine personal care needs, safety concerns, and best practices to foster independence at maximum level of function. The Wellness Coordinator is a licensed nurse specializing in geriatric care. This focus on maintaining clinical well-being is achieved through recognizing and intervening on the changing needs of seniors, communication with physicians, and through staff education on new medications or other medical practices. Joining this team is a Therapeutic Programming Coordinator who assesses interests and abilities and assists in structuring activities, leisure time and programming which assures days are purposeful, meaningful and personalized to each resident. The Staff Services Coordinator trains all the resident assistants on the individual care each resident requires. They mentor new staff, guide in-servicing opportunities and oversee the daily routine in our communities. As the team meets together, they discuss each resident and prepare an individualized service plan (ISP) for each person. When changes occur, they look at the many factors which could be causing concern. Creative solutions are found to enhance the well-being of our residents so they may maintain the highest standard of living and get the most possible enjoyment out of their days.
